  • IMPORTANT: New Process to request a COI, for fields NOT owned by your Club/Team/League, for any sanctioned EPSA match or competition.
    1. Contact the facility/venue and request, in advance, a contract or field permit indicating the dates, times and location of the field you want to use

2. Email or mail the signed contract/field permit along with the COI request form to the State Office.
3. Upon approval, a COI will be issued and a copy sent to the individual making the request.
